Worm suffocated itself in a block.
Long description:
Pretty straightforward. The worm boss got suffocated in a block. Im pretty sure it happened because it was a block on top of a fence. Like it was a cobblestone gate block or fence block and above that it had a full solid block. It went into the fence which caused the block on top of the fence to suffocate it. It stayed on there for like 15 seconds before dying.
